703 Welcome to Smart health Patient engagement service. Our medical assistants and support staff provides patient engagement service that can result in patients who are better in touch with their health and follow the doctors protocol. Our engagement agents work with the patients to self-monitor themselves and enabling everyone involved with their health to better understand the patient personal needs. Together they will establish a base line of the patients vitals that will be compared to measure if the patient health is improving or deteriorating. Each engagement agent is assigned to a list of patients. And out side of the registered nurse this agent will be the go to person for any needs the patient may have. The agents are performance is measured by their percentage of patients who follow protocol and stay engaged. Their performance has bonuses based on the star rating of their population of patients.
